Here are some quick recourses for you regarding treatment/post-treatment expectations:
We want to make your experience with Southern Illinois a comfortable and pleasant one. Scheduling an initial consultation is as easy as a phone call or email request. Here is what you can expect from your first visit:
Medical History and Information
The initial visit allows our patients and staff get to know one another. You will be asked to complete a comprehensive medical history with a focus on your venous history. This can be completed prior to the appointment, using the document links below.
Physical Exam
We will give you a complete physical exam, which may include a screening ultrasound to diagnose your specific varicose disease. The precise diagnosis will tell us which treatment options may be most effective for your individual case. Photographs of your legs may be taken to allow us to track your treatment progress. Some insurance companies require copies of these photos as part of their pre-authorization process.
Discussion of Treatment Options
After the exam, we will discuss your best treatment options. Some treatments require more than one session to achieve desired results and we will talk about this possibility. We will also go over procedures in detail so you know exactly what to expect from subsequent visits. It is important for you to feel comfortable with the prescribed treatment, so feel free to ask any questions that come to mind.
To make your first session as productive as possible, please bring the following items with you:
• A comfortable pair of loose fitting shorts for the exam
• The name and phone number of your primary physician
• Your insurance card
• Any forms completed prior to the initial consultation
For the convenience of our patients, we provide PDF versions of our required patient forms. You can print these at home and bring completed copies to your first visit:
Before your next appointment, we will work with your insurance company to determine coverage. If there is an out of pocket expense, we will provide that information and payment options to you prior to the treatment.
Please note that we WILL be able to see you even if you don't bring all of the above items to your first visit.
